Please provide a detailed description of any incidents (state all facts, events and circumstances of any such incident relating to the match even if it occurred before or after the match or during half time).


The Send-Off Report must be submitted by 9am on the day following the match.

An Incident Report is to be used for incidents such as:

  • Post Send-Off incidents (e.g. After you give a player an R2 he/she verbally abuses you);
  • Spectator misbehaviour;
  • Issues in regards to the facility (lighting or change rooms)
  • Postponements, abandonments, forfeits; or
  • Serious injuries (for example where an ambulance attends).
